Men's health telehealth questions

Straight answers on eligibility, cost, and how care works. If yours isn't here, email us.

Who can become a patient?

Axiom serves adults 18 and older who are physically located in Washington State at the time of each visit and have a Washington address for prescriptions and lab work. Axiom is a telehealth-only practice. Our clinician is licensed in Washington, so we can only see you while you're in Washington, even if you're only away for a short trip. If you're traveling outside Washington, we'll reschedule your visit for when you're back.

Do you take insurance?

No. We're a cash-pay practice and don't bill insurance. Your invoice lists itemized services. You may be able to use HSA or FSA funds for some services, but membership fees may not qualify; check with your plan.

What does it cost to start?

To start, you pay for your first labs (at our cost, no markup) and a $99 non-refundable deposit toward your first video visit, together: $231.71 with Quest or $365.90 with Labcorp. If we've already accepted your recent outside lab results, you pay only the $99 deposit. Once your results are in, we'll send you a link to book the visit, with nothing more to pay. The deposit isn't refundable if you change your mind or don't complete your labs, and your visit is always offered. We refund it in full if we cancel or can't provide your first visit, or if we find after you pay that you aren't eligible for a non-medical reason. If you don't complete your blood draw, ask us within 60 days of your payment and we'll refund your lab payment. The deposit isn't applied toward membership. Membership starts at $49 a month if you start treatment. See our Membership page for prices, our Labs page for lab costs and our Cancellation Policy for deposit terms.

What does membership actually include?

Visits, secure messaging, prescription management, lab review, and the follow-up labs on your treatment schedule. Medications aren't included in membership. You pay for them separately.

How do I cancel my membership?

You can cancel in your patient portal, by emailing info@axiomperformancemedicine.com, or by calling 206-880-0522 and leaving a voicemail. Portal cancellations take effect right away: your membership won't renew, and you keep access through the end of the month you've already paid for. Email and voicemail requests are processed within 1 to 2 days, and your cancellation counts from the day we receive your request. If a renewal charge goes through in the meantime, we'll refund it. Refund details are in our Cancellation Policy.

Can I combine treatments?

Yes, and combined memberships cost less than adding them separately. TRT plus ED is $159 a month, for example, and Axiom Complete (TRT, weight, ED, and hair) is $249. The full list is on the Membership page.

What is founding member pricing?

The first 25 members get Founding TRT at $109 a month (regular $129) or Founding Axiom Complete at $219 (regular $249). Founding spots are limited to 25 total across TRT and Axiom Complete. The rate stays with you as long as your membership is continuous.

Which lab do you use?

We order labs through Fullscript, and you get them drawn at a participating lab in Washington. Before we order any lab you'll pay for, we'll tell you the price.

Who pays for labs?

Your membership covers the follow-up labs your clinician orders on our standard schedule. You pay for three kinds of labs: your first labs, labs after a later dose change (for TRT), and any add-on labs. You pay our cost, with no markup. Before we order any lab you'll pay for, we'll tell you the price.

Do you accept outside lab results?

Yes. You can send us lab results from another provider or from a lab-testing service you ordered yourself. If you're starting treatment with us, the sample must have been collected within the last 3 months. If you're already on treatment and transferring your care to us, results collected within the last 6 months can be used. If any of those results are outside the lab's normal range, your clinician may ask you to repeat them. Results must come from a CLIA-certified clinical laboratory and show your name, the collection date and the lab's name. Please send them through your secure patient portal, not by email, text or social media.

We don't accept results from at-home finger-prick test kits, because we can't confirm how the sample was collected.

Your clinician decides whether the results you send cover what's needed for your care. If something is missing or out of date, we'll tell you which labs to complete. Before we order any lab you'll pay for, we'll tell you the price. Sending outside results doesn't change which labs your membership covers.

Do I need labs before a refill? Can I use labs I already have?

Your clinician may need updated labs before some refills or changes to your plan. You can send results from another provider through your secure patient portal if they come from a CLIA-certified lab. To start treatment, the sample must have been collected within the last 3 months. If you're already on treatment and transferring to us, results from the last 6 months can be used. If any of those results are outside the lab's normal range, your clinician may ask you to repeat them. We don't accept at-home finger-prick kits. Your clinician will tell you whether your results can be used. Before we order any lab you'll pay for, we'll tell you the price.

How do visits work?

Five steps, typically in this order. 1. Intake and consent. Answer a few eligibility questions, then review our privacy notices and complete your consent forms and health history in our patient portal (Practice Better), including your current medications and what you've tried before. 2. Free info call (optional). At the end of your intake, you can choose a short call about how Axiom works, or skip it. This call isn't a medical visit. Medical advice and treatment decisions happen with your clinician. 3. Labs. Before any labs are ordered, you'll get a Good Faith Estimate of expected costs. If labs are appropriate, your clinician orders them. You pay for your first labs at our cost, with no markup, together with a $99 non-refundable deposit toward your first visit. You get them drawn at a participating lab in Washington. 4. Video visit (already paid by your $99 deposit). Once your results are in, we'll send you a link to book a video visit with Timothy Shawn Dannels, ARNP, FNP-BC to review your results and options. Nothing more to pay at booking. Treatment is prescribed only if your history and labs support it. 5. Membership, only if you start treatment. Membership renews each month until you cancel. Medications aren't included in membership. You pay for them separately.

Who is the prescribing clinician?

Timothy Shawn Dannels, ARNP, FNP-BC, a Washington-licensed advanced registered nurse practitioner (ARNP) and board-certified family nurse practitioner (FNP-BC), is the clinical founder and prescribing clinician. Brandon Morris, administrator, leads patient experience and operations.

Will you prescribe medication at the first visit?

Only if the history and labs support it. For testosterone, that means symptoms plus low levels on properly timed early-morning lab testing, and a look for reversible causes first. If treatment is right for you, your clinician sets and adjusts your plan based on your labs, your symptoms and regular follow-up.

I had a low testosterone result elsewhere. Does that mean I need TRT?

Not on its own. A diagnosis needs symptoms plus low total and free testosterone on properly timed early-morning lab testing with a reliable assay, and a look for causes that can be corrected. We'll review any prior results and decide together whether treatment makes sense.

How often are labs drawn on treatment?

After you start treatment, we typically repeat labs at 6–8 weeks, again about 6 months in, then about every 6 months with a yearly comprehensive panel, as your clinician directs. These are included in membership. Blood pressure and symptoms are reviewed at check-ins.

What testosterone levels do you aim for on treatment?

We aim to relieve your symptoms, keeping your testosterone at a level your clinician sets based on your labs and symptoms, using the lowest effective dose. Your clinician adjusts your dose based on your symptoms and follow-up labs, and may lower it if your levels or side effects suggest it's too high. Supraphysiologic, meaning above-normal, levels aren't the goal and can add risk.

Can I stay fertile on testosterone?

Testosterone therapy typically suppresses sperm production. If you may want children, tell us before we start. There are approaches that preserve fertility, and we'll discuss which fits your situation.

Can Axiom be my primary care provider?

We're a specialty performance and prevention practice, and we coordinate with your other clinicians. We're not an emergency service and not a full-spectrum primary-care home for every problem. Keep a primary care provider.

How do I get started?

Axiom serves adults who are physically in Washington during each visit. To begin, start your intake in our secure patient portal. At the end of your intake, you can choose a free info call (optional) with Brandon, our Administrator, who answers questions about how Axiom works, or skip the call and move on to your first labs once your intake is reviewed. This call isn't a medical visit. The call itself is free. If you decide to go ahead, you pay for your initial labs (at our cost, no markup) together with a $99 non-refundable deposit toward your first video visit.

How fast will you reply?

Send us a message in your patient portal. We aim to reply to messages and return calls within 1 to 2 days. Phone and telehealth hours: 9 AM–8 PM PT, 7 days. Messages aren't monitored around the clock, so don't use them for urgent needs. For emergencies, call 911.
