Consumer Health Data Privacy Policy
Effective date: October 17, 2026 Last updated: October 24, 2026
This Consumer Health Data Privacy Policy explains how Axiom Performance Medicine, PLLC, a Washington professional limited liability company owned by Timothy Shawn Dannels, ARNP, FNP-BC ("Axiom," "we," "us"), collects, uses and shares consumer health data under Washington's My Health My Data Act (chapter 19.373 RCW), and how you can exercise your rights.
Patients: Health information we collect while providing care to you as a patient is also protected by HIPAA and is described in our Notice of Privacy Practices. Where HIPAA applies, that Notice governs. This policy covers consumer health data that HIPAA does not, such as information from our website, contact forms, email and social media before you become a patient.
1. Consumer health data we collect, and why
"Consumer health data" means personal information that is linked or reasonably linkable to you and that identifies your past, present or future physical or mental health status (RCW 19.373.010). That can include the fact that you are seeking health care services.
| Category | Examples | Why we collect it and how we use it |
|---|---|---|
| Health services you're seeking | Which service you ask about (testosterone care, weight management, sexual wellness, hair restoration), booking requests | To answer your question, check eligibility (adults located in Washington), and schedule a visit |
| Health information you send us | Symptoms, conditions, medications or goals you mention in a contact form, email or message | To respond to you and decide whether our services may be a fit |
| Identifiers linked to the above | Name, email, phone number, date of birth, state of residence | To contact you, confirm you're an adult in Washington, and keep records of your request |
| Device and usage data linked to health pages | IP address, browser and device type, and the page requested, which our website host (Netlify) receives in standard server logs when you load a page and which Google receives when your browser loads our web fonts from Google Fonts | To operate, secure and deliver the website. We do not use this data for advertising, profiling or analytics, and we don't combine it with patient records. |
We use consumer health data only for the purposes listed above, or for purposes you separately consent to.
2. Where we get it
- Directly from you: website forms, booking requests, email, phone calls and messages.
- Automatically from your device: when you use our website (see section 1, last row).
We do not buy consumer health data.
3. Consumer health data we share, and with whom
We share consumer health data only as needed to provide the services you ask for, and only with your consent unless the law otherwise allows it.
| Category of data shared | Category of third party | Why |
|---|---|---|
| Identifiers and the services you're requesting, plus health information you choose to send us | Our patient scheduling, intake, records and secure messaging platform (Practice Better) | To book and manage your visit |
| Identifiers and messages | Our email provider (Google Workspace) | To respond to you |
| Identifiers, voicemail and call details | Our business phone and voicemail provider (Google Voice, through Google Workspace) | To return your calls and messages |
| Website technical data (IP address, browser/device, page requested) | Our website host (Netlify) and our web font provider (Google Fonts) | To deliver and secure the website |
| Identifiers, health history, lab orders and results, and prescription information | Fullscript (lab ordering), clinical laboratories (Quest, Labcorp or another lab that processes your tests) and pharmacies that fill your prescriptions | Shared only as necessary to provide the care you request |
Affiliates: Axiom has no affiliates and shares no consumer health data with affiliates. Specific processors: Practice Better (patient scheduling, intake, records and secure messaging) · Netlify (website hosting) · Google (Google Workspace, email; Google Voice, business phone and voicemail) · Fullscript (lab ordering). Other third parties: Google Fonts (web fonts; see section 5) · clinical laboratories (Quest, Labcorp or another lab that processes your tests) and pharmacies that fill your prescriptions: shared only as necessary to provide the care you request. Telehealth video visits use Practice Better's built-in video. If you opt in, Practice Better sends our appointment and portal-alert texts (SMS); texts don't include health details. Reply STOP to opt out. Our website has no contact or intake forms. Booking and health questions go through Practice Better's secure portal.
Our processors may handle consumer health data only on our instructions and under a contract (RCW 19.373.060).
Once you become a patient, our Notice of Privacy Practices governs how we use and share your health information.
4. We do not sell consumer health data
Axiom does not sell consumer health data. We will not sell it without your signed, valid authorization as required by RCW 19.373.070.
5. No third-party advertising pixels
Our website does not use third-party advertising pixels, analytics or tracking tools, such as the Meta Pixel, TikTok Pixel, Google Analytics, Google Tag Manager, Google Ads tags or similar, and it sets no tracking cookies. We do not share your website activity with advertising platforms.
The only third parties that receive data when you load our website are our host, Netlify, which receives your IP address and standard request information in order to serve the page, and Google Fonts. Our website loads its fonts from Google Fonts, so when a page loads, Google receives your IP address, browser information (user agent) and the address of the page you're viewing. Google states that it doesn't use this information to create user profiles or for targeted advertising, and that the Google Fonts API sets no cookies. See the Google Fonts privacy FAQ.
If we ever add tools like these, we'll update this policy first and get your separate consent before any consumer health data is shared.
We do not use geofencing around health care facilities (RCW 19.373.080).
6. Your consent
We collect consumer health data only with your consent, or as needed to provide a product or service you ask for (RCW 19.373.030).
- Consent to collect: when you submit a form or booking request, we ask you to agree that we may collect the consumer health data in your submission for the purposes in section 1.
- Separate consent to share: we ask for a separate consent before sharing consumer health data with anyone other than our processors, except where sharing is necessary to provide the service you asked for.
Consent requests are kept separate from our other terms. You can withdraw consent at any time (section 7).

- Confirm and access: confirm whether we collect, share or sell your consumer health data, and get a copy of it.
